Delivering integrated health and social care for older people with complex needs

The Welsh Government’s framework for delivering integrated health and social care for older people with complex needs was published in march 2014 and provides a national blueprint for service development and delivery aimed at ensuring that frail elderly people in Wales have a strong voice and control over their care and support, that prevention and wellbeing are at the heart of those services and that provision is coordinated across agencies to deliver the outcomes that are important to people and their carers.

In common with all areas of Wales, Hywel Dda University Health Board and its partner local authorities were required to produce Statements of Intent assessing current arrangements against the standards set out in the Framework and setting out commitments for taking integration further in the coming period. The Statement has been ratified by all partners and will be used to plan activity and monitor our progress as we work together to deliver our commitments and improve outcomes for older people in our area. We will publish regular updates on progress as we proceed on this journey.
