We have a shared vision with you and your Community for safe, sustainable, and accessible Health and Care services provided as close to your home as possible. This is part of our strategy for 'A Healthier Mid and West Wales'.

We want to talk with you about how this could be achieved in your local area.

What are we doing?

We are planning for how we best provide Primary Care and Community-based services in Carmarthenshire, Ceredigion and Pembrokeshire.

These are most of the NHS provided health and care services in your area, apart from hospital-based services.

Health and care can be delivered by the NHS directly, by Contractors (such as those listed below), by Partners such as Social Care, or by grass-roots Community groups. Find out more about this below.
