Continuing NHS Healthcare

Our long term care team works across the counties of Carmarthenshire, Ceredigion and Pembrokeshire. They work in partnership with three local authorities, mental health teams, community teams, voluntary and third sector organisations and independent nursing homes.

There are currently over 25 nursing homes in the area. They provide a mix of general nursing care and dementia care.

The team manage processes for long term care pathways such as:

Continuing NHS Healthcare (CHC)
NHS Funded Nursing Care (FNC)

The process is started when a health professional makes a referral to the long term care team. The team then starts the assessment process to confirm if the patient would be eligible for Continuing NHS Healthcare.
