Your clinician may have advised you that you are a “Patient Initiated Follow Up symptom” patient. This means that you have a long term condition but a routine follow up appointment is not required for a number of reasons. It could be your treatment has been completed or your condition is being well managed and no further clinical intervention is required at this time.
However, if you experience any problems with your treatment you can still contact us. Your clinician will have advised that you can contact us and ask for a “patient initiated follow up’ appointment.
Before being discharged, you will have been advised what symptoms to be aware of. If you have any concerns about your condition or experience a ‘flare up’ of your symptoms, you need to contact your clinical team.
You can do this either using the telephone number on the letter or through our Contact Centre.
Monday to Friday from 8.00am until 6.00pm (excluding bank holidays)

The call handler will then arrange for a member of the clinical team to contact you to discuss your symptoms. They will provide advice and guidance on what to do next.
They can arrange for further tests, if required, or book an appointment for your treatment or condition to be reviewed.
“Patient initiated follow up” is designed to support and encourage you to co-own your health care. It will help support you to self-manage your care decisions and share the decision making with your clinicians.
It also helps us to free up appointment availability for patients, so you’re seen at the right time.
