Memory assessment service

The memory assessment service is a primary care diagnostic pathway hosted by the older adult mental health service (opens in new tab) for people of any age with a suspected dementia. The service offers:

  • timely and sensitive diagnosis for people with dementia
  • tailored information and education
  • psychological and medical treatment dependant on need
  • a plan of care and post diagnostic support before return to the care of a GP

The teams are led by advance nurse practitioners, one in each county, with a dedicated medical lead consultant psychiatrist, consultant neuropsychologist and includes specialist memory assessment nurses and occupational therapists.

Referrals

To rule out common reversible causes of memory impairment, we ask that a GP complete a physical health screen. This is to rule out any other possible causes for changes in your memory prior to you being referred to the service.

Assessments

Assessments are usually undertaken with a clinic environment and take approximately two hours and diagnosis is mostly complete with 12 weeks from referral.
