The memory assessment service is a primary care diagnostic pathway hosted by the older adult mental health service (opens in new tab) for people of any age with a suspected dementia. The service offers:
The teams are led by advance nurse practitioners, one in each county, with a dedicated medical lead consultant psychiatrist, consultant neuropsychologist and includes specialist memory assessment nurses and occupational therapists.
To rule out common reversible causes of memory impairment, we ask that a GP complete a physical health screen. This is to rule out any other possible causes for changes in your memory prior to you being referred to the service.
We have an additional service level agreement with the Alzheimer's Society who dedicate additional support during the assessment and diagnostic process. Their community-based dementia advisers provide information, guidance, and practical support. They help people understand their symptoms, cope with day-to-day living, navigate health and social care systems and prepare for the future.
Assessments are usually undertaken with a clinic environment and take approximately 2 hours and diagnosis is mostly complete with 12 weeks from referral.
