We provide specialist NHS mental health assessment, treatment and care for older people facing functional mental health difficulties in later life. We also deliver significant components of the West Wales Dementia Wellbeing Pathway. We aim to enable older people in West Wales experiencing adverse mental health to age-well, experience health equality, recover their wellbeing, right to self-determination and independence.
We operate in both community and hospital settings Hywel Dda with extensive multi-professional/ inter-agency collaboration across many services.
Older Adult Community Mental Health Teams (OA-CMHT) are a secondary specialist multi-disciplinary assessment, care and treatment service. They work with older people, their family and carers, whose age related mental health needs, or dementia, due to complexity and risk, cannot be fully addressed by their GP and/or other primary care support services.
The OA-CMHT operates between the hours of 9am - 5pm, Monday - Friday (where necessary people with functional mental health needs are additionally supported out of hours by the crisis and home treatment teams).
