The below is a request for help form used to request the help of the Children and Young People’s Physiotherapy service. This will help us to identify whether our service is indeed the most appropriate to be providing this help.
We would encourage you to complete these forms with your child or encourage them to complete the form themselves if you feel this is appropriate. Please send any “request for help” forms to the relevant address for your area (area sections below).
If the request for help is accepted you will be contacted with an appointment. If we are not sure whether our service is appropriate we may contact you or the referrer for more information. If we feel our service is not the most appropriate you will receive a letter to inform you or we may sign post you to a more appropriate service.
